Should we enter ledgers directly as opposed to having one to one communications and the setting them on a ledgers?

We mostly have one to one relations when the outcome we are looking for (a new loan, registration of a kid in school, etc.),depends on a process. And such process is then reported on paper or is/ becomes digital.
